The purpose of this role is to work as part of a multi-disciplinary team that provides mental health support to young people transitioning from Children’s Mental Health services to Adult Mental Health Services. To follow a “person centered” model of service delivery focusing on the direct needs of the service user, working across boundaries of care, organization and role.

Roles of Mental Health STR Worker:

  • To be an integral member of the Young Person’s Transition Pathway, working with individuals who are about to transition from CAMHS, delivering interventions under the instruction/supervision of the Young Persons Mental Health Pathway Lead.
  • To engage the identified individuals and to provide support and interventions to meet their health and social needs through individual work
  • Support and work with the team to improve the interface between services for people with mental health difficulties, such as housing/financial wellbeing.
  • Work with the Mental Health Practitioners in the CAMHS and AMHS to provide tailored individual care and support plans describing anticipated treatment/s and support packages that promote recovery.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care including this support to care-leaver and students.

Profile

  •  NVQ Level 2 and 3 in Health Care/Mental Health/Social Care/Health Science
  • Certificate in Mental Health or Commitment to work towards achieving it.
  • Additional higher level qualification such as diploma or NVQ level 3
  • Evidence of other up-dating and development in social/mental health care
  • Experience of working with people with mental health needs.
  • Providing support to individuals and carers.
  • Supporting people with activities of daily living and promoting self-care, social functioning, and independence.
